What is Know Your Numbers all about?
Know Your Numbers is a foundational part of the Wellness 360º program. It's a health screening service that focuses on self awareness and personal health management. You can find all the information you need to participate at (URL). By attending a Know Your Numbers health screening, you also get access to 12 months of professional, one-on-one health coaching. Plus, if you are enrolled in Limeade™, you will be able to include your Know Your Numbers information within your personal and confidential Limeade™ account. This helps complete the Limeade™ picture of your health and well-being.

Why are the Know Your Numbers screenings important?
By attending a health screening, you will learn important information about yourself—you will learn your biometric numbers. It's a first step in learning how to improve your health and well-being. In most cases, you will never know your numbers unless you are tested. That's why Wellness 360º is offering this service. And, with 12 months of follow-up health coaching, you have an advantage as you continue to take steps to improve your health.

Why should I take part in this if I already lead a healthy lifestyle?
Good for you! Participation in the Know Your Numbers service is entirely voluntary. Since you're already choosing healthy habits, you likely do not have health risks that are controllable. The screening can reveal important health risks—some of which are influenced by other factors and are not always apparent. We are making this service available to increase self awareness of your current health status and what you can do to reduce your risk.

Who is the Know Your Numbers vendor?
L&I is partnering with an experienced health screening company called Worksite Wellness, LLC www.worksitewellness.net. Worksite Wellness, LLC is located in Redmond, Washington.

What screening measurements are taken?
The professional health technicians and health educators from Worksite Wellness, LLC will collect the following measures: height, weight, and blood chemistries of total cholesterol, HDL cholesterol, LDL cholesterol, triglycerides, and glucose. Blood is collected using a finger prick method.

Can't I get this same information through my own doctor?
Yes, that's true. But, with Know Your Numbers, it's usually more convenient. Of course, you may want to share your Know Your Numbers information with your own doctor.

How long does a screening appointment take and when is the health screening happening?
It's takes about 20-25 minutes for a health screening appointment. Health screening appointments are being offered to L&I employees during the summer and early fall of 2010.

What do I need to do to prepare for the screenings?
The cholesterol/glucose test is a simple finger-prick test that will provide you with immediate results. We do ask that you refrain from consuming any food or beverages (other than water) 4 hours prior to your test. This will help ensure a more accurate reading.

Tell me more about the health coaching service.
If you participate in Know Your Numbers you are eligible for health coaching. Health coaching is the perfect choice for long-term follow-up and support for those wanting to make changes for the better. The health coaching vendor is Well Call, www.wellcall.com, a HIPPA-compliant health promotion company based in San Francisco. The coaches are professional health experts specifically trained in one-on-one telephone coaching. Coaches can assist employees with exercise and fitness, weight management, pre and postnatal care, tobacco cessation, management of chronic conditions, menopause, stress management, and complementary care including yoga, massage, acupuncture, and chiropractic care. You will be given the health coaching toll-free phone number and customer code upon the completion of your Know Your Numbers health screening. The coaching process is initiated by you. Most employees find that weekly phone coaching sessions work best. Coaching phone calls tend to last from 10-20 minutes in length. You are to use the health coaching service during your own time such as breaks, lunch, and before and after work.

How do I sign up for the Know Your Numbers health screening?
You sign up directly with the vendor using a confidential online registration system set up just for L&I employees. You will receive a reserved appointment time slot.

What if I can't attend an onsite health screening session?
If you miss the session at your office, you can attend a session at another L&I office. If you cannot attend a scheduled onsite session, you are invited to sign up for a take-home test kit. The test kit includes a finger prick device. There is no cost to you for the take-home test and 12 months of health coaching is included.

How long does it take to do the home test kit and get my results?
It typically takes 3 to 5 business days to receive the kit and another 10 to 15 days for your results to be processed at the lab and posted online. All test kits are shipped by first class mail via USPS. You will receive an email message notifying you when your results are available. This message will contain instructions for viewing your results and printing your lab report.

Tell me more about the protection of my privacy.
Worksite Wellness, LLC uses strict privacy protocols during the health screening process. When needed, privacy screens are used and private conversations between the professional screening staff are handled in a discrete manner. Wellness 360º partners only with 3rd party health care providers who guarantee the secure and confidential handling of your personal health information in compliance with HIPAA. Any personal health information that may be collected with Know Your Numbers is stored on the secured servers of Limeade™, another L&I 3rd party provider. At no time does L&I have access to your personally identifiable health information.

How will the data be used?
Only you will see your individual results. Know Your Numbers is intended to increase your awareness of personal health risks so you can better manage your health. No personal data are shared with L&I. However, group aggregate data will be available to L&I. This will allow Wellness 360º to identify specific health management programs that may be appropriate for the L&I employee population.
